STATE COLLEGE — Juniper Village at Brookline Rehabilitation and Skilled Care has received a five-star overall rating from the Centers for Medicare & Medicaid Services, the highest possible designation under the CMS Five-Star Quality Rating System.
The recognition reflects the community’s continued commitment to providing high-quality care while creating a supportive environment where residents can maintain their independence, stay connected, and receive the personalized support they need.

The CMS Five-Star Quality Rating System is a nationally recognized tool designed to help residents and families compare skilled care communities and make informed care decisions. Overall ratings are based on three areas: health inspections, staffing, and quality measures. A five-star overall rating indicates that a community’s performance is considered much above average compared with other skilled care communities nationwide.
