An 87-year-old woman had to be taken to the hospital after disappearing from her assisted-living facility for several hours.

Police say the woman wandered away from the Juniper Village at Brookline assisted living facility on Thursday afternoon. Staff at Brookline contacted police when they noticed she was missing, leading to a search that lasted for about three hours.

Members of the Centre Region Emergency Strike Team found the woman nearby “in a hypothermic condition,” according to police. She was transported to Mount Nittany Medical Center by EMS.

Police could not release the woman’s name or confirm her current condition.
